MCYSA REGISTRATION ROSTER:  - Coaches must download this form, complete the information, print it off,  and include the printout in their Carding Packets submitted to the DC for verification.  In lieu of having each family complete the form it is a roster for the team.

The request for a family email address, preferably the parents', applies only to age groups, U10, U11, U12, U13, U14, U17, U18 and U19. Completion of the form posted on the web site is mandatory for each team in these age groups. No player in these age groups will be carded unless the information is supplied. The information must be given to the DCs at the time the packet is submitted for carding. The information requested on the form can be given to the DCs in paper form or on a computer disk. If a family does not have an email address, the family should supply the email address of someone who will be able to receive an email and get the information to the family. The purpose of the gathering of this information is to insure that families are notified of the existence of the Select Program and the Junior Showcase and of the availability of the two $1,000.00 Keegan and Rinier Scholarships. These are the only purposes the league will use this information for. The league will not disclose this information to third parties.
