Travel Player Forms

Constitution and Bylaws - These are the Constitution and Bylaws of Bridgewater United Soccer Club.
MCYSA Coach - KidSafe Disclosure Form - Updated for 2009-2010 seasonal year, previous year forms may be used
MCYSA Coach - Membership Form - Updated for 2009-20010 seasonal year, previous year forms should not be used
MCYSA Non-Volunteer (Paid Coach) - Non-Volunteer Membership Form This form can only be completed through the NJYS Office. District Commissioners cannot issue a Coaches pass to a Non-Volunteer. Packages for registering Non-Volunteers can be obtained from the NJYS office. The completed package must be returned to the office for processing which may require a week to complete. Non-volunteers cannot have their paperwork completed while they wait.

MCYSA Player - SAGE form - To remind us that the game is for the kids, that respect for others is a lesson we can help teach, and that Setting A Good Example is more important than winning. Parents, Coaches, and Players sign Sportsmanship Pledge forms spelling out appropriate conduct and the reasons why good sportsmanship is so important. Parents and Player(s) from one family can all sign the same form. Every team carding packet submitted must include a signed form for all players and coaches to be carded
